高迁移速率蛋白和终末期氨基糖基化受体在退行性主动脉瓣膜钙化中的表达及临床意义

Clinical correlation and expression of high mobility group box 1 protein and receptor of advanced glycation endproducts in patients with calcific aortic valve disease

摘要: 目的 检测高迁移速率蛋白(high mobility group box 1 protein,HMGB1)、终末期氨基糖基化受体(receptor of advanced glycation endproducts,RAGE)在退行性主动脉瓣膜钙化(calcific aortic valve disease,CAVD)病人瓣膜中的表达,并探讨其与CAVD病人发生发展及临床特征的相关性。方法 通过免疫组化法检测病人瓣膜组织中HMGB1及RAGE的表达,分析其表达与瓣膜钙化的临床特征及血清中甘油三酯、总胆固醇等表达的相关性。结果 HMGB1在CAVD病人组织中的表达率为62.3%,正常瓣膜组织为6.3%,两组比较差异有统计学意义(P<0.05),RAGE在CAVD病人瓣膜中的表达率为53.6%,正常瓣膜组织为31.3%,两组比较差异有统计学意义(P<0.05);相关性分析显示,CAVD病人瓣膜组织中HMGB1和RAGE的表达呈负相关(r=-0.363,P<0.05)。 HMGB1及RAGE的表达与总胆固醇、低密度脂蛋白、年龄、瓣膜钙化程度相关(P<0.05)。结论 CAVD病人组织中HMGB1及RAGE高表达,HMGB1与RAGE 的表达与瓣膜钙化程度正相关。在CAVD瓣膜组织中,HMGB1、RAGE可能是有利于临床判断CAVD程度及指导临床治疗的生化指标。

Abstract: Objective To investigate the expression of HMGB1 and RAGE in calcific aortic valve disease(CAVD)and normal valve tissues,and to investigate the correlation between HMGB1 and RAGE in the occurrence and development of CAVD.Methods The expression of HMGB1 and RAGE was detected by immunohistochemical method in CAVD valve tissues and normal valves,and the relationship between the expression level of CAVD and clinical pathological features of CAVD were analyzed.Results The expression of HMGB1 was 62.3% in CAVD and 6.7% in normal valve tissues(P<0.05),and the expression of RAGE in CAVD valves(53.6%)was significantly higher than that in normal valve tissues(31.3%)(P<0.05).HMGB1 and RAGE expression were correlated with the total cholesterol,triglycerides,ox-LDL and calcific score(P<0.05).Correlation analysis showed that the expression of RAGE and HMGB1 in CAVD was negatively correlated(r=-0.363,P<0.05).Conclusion The expression of HMGB1 and RAGE in CAVD valves is higher than normal valves.The expression of RAGE decreases with the increase of HMGB1 expression,suggesting higher stage of calcific score.In CAVD,RAGE and HMGB1 are biochemical indicators that are helpful to judging the degree of calcific valves and guiding the clinical treatment.
